Recognizing the staff, physicians and volunteers for the great work they do is extremely important to the Perth and Smiths Falls District Hospital and we are proud to promote the excellent high quality care that is provided to our community.
Did you know that the Perth and Smiths Falls District Hospital has a recognition program called, "The Spotlight Awards"? The awards provide recognition to our staff, physicians and volunteers who have made a difference in the hospital experience for our patients and their families.
Recognition can come from a patient, family member, visitor or colleague. Simply submit your note of recognition online by completing the form below; printing a form or requesting a hard copy from personnel on each nursing unit. Completed hard copy recognition forms can then be submitted in person or to spotlightawards@psfdh.on.ca.
The names of
each recognized person will be posted on our internal quality white boards and as well on
our website and through the hospital social media pages.
