Please try to have a medication review printout from your community pharmacist the week before any planned admission or surgery. Bring this review with you to the hospital. As well, please bring all of the medications that you are taking including prescriptions and over the counter medications such as Tylenol, vitamins and herbal products that you take, whether occasionally or on a regular basis. 

Please ask a member of your healthcare team for a blue medication pocket card.
