Hospital 2018-2019 Quality Improvement Plan Now Available

Report on Progress Strategies from 2017-2018

SMITHS FALLS, ON – Perth and Smiths Falls District Hospital (PSFDH) has prepared its 2018-2019 Quality Improvement Plan for submission to Health Quality Ontario (HQO).  It is a blueprint to focus on initiatives for the coming 12 months.

A Quality Improvement Plan (the Plan) is a formal commitment to quality improvement, aligned with system and provincial priorities. By identifying and explaining how they are going to achieve their long-term improvement goals in their Plans, Ontario’s health organizations are able to effectively focus their quality improvement efforts on those key issues that will truly improve health care in our province.

According to last year’s Plan, 4/7 of PSFDH target areas were achieved and or improved. The other 3/7 targets remain priorities of this year’s Plan.

“Our Quality Improvement Plan is focussed on creating a positive patient- and family-centred experience and delivering high quality health care.  With this approach, we are able to set and monitor our targeted areas for improvement,” said Beverley McFarlane, President and CEO.  “This Plan also builds upon the progress achieved over the recent year plan.”

The PSFDH is an active participant and leader that supports and ensures Ontarians receive healthcare of the highest quality possible. The hospital is focused on creating and maintaining a positive patient experience and delivering safe and high quality health care. Great strides have been made over the past year in embedding the patient and family voice throughout the organization.

Quality Improvement Plans are mandatory in Ontario and publically posted on hospital websites. The working document is developed by hospital leaders and staff and requires the approval of the Board of Directors prior to submission to the South East Local Health Integration Network and Health Quality Ontario.

The Plan consists of a progress report (outlining results for previous year’s plan) a work plan and a narrative to support the upcoming year’s goals and objectives. All staff and physicians are involved in the success of the plan.

The Plan includes five Aims (effective, efficient, patient centered, safe and timely) as well as some substantive areas:

    • to address key safety improvements specifically preventing workplace violence and ensuring safe narcotic practices;
    • to continue enhancements to patient and family centred care initiatives and improve patient satisfaction, with a focus on the Emergency Departments; and
    • to provide effective transitions upon discharge from hospital.

PSFDH’s Quality Improvement Plan aligns with the current organizational Strategic Plan, the Hospital Services Accountability Agreement and other community initiatives such as Health Links and Health Care Tomorrow.
